CAPOX first or surgery first?

Hi everyone, hope you’re all doing alright!

I’ve had chemoradiation for my stage 3 lower rectal tumor, it shrunk it to 2cm away from the margin for APR or LAR which I would prefer.

my tumor board recommended 4 months of CAPOX then possible surgery, my oncologist says he doesn’t think it’s a good idea and is urging me to get APR surgery then CAPOX

I think the CAPOX first would be the better way to go, any thoughts??

  • Hi  The recent Foxtrot trial showed that having chemo before surgery decreases the chance of it coming back. If you have chemo before your op then you’re in a better physical shape to deal with it whereas it can be tougher after an operation especially if recovery takes longer than expected.

Personally I agree with you that chemo before the op is preferable unless your surgeon has a very good reason for delaying it until after?

    Take care
