Bowel cancer

Hi,

I was diagnosed with bowel cancer in June this year. Only discovered because i was having terrible stomach aches / diarrhoea / sickness etc during the whole of May. After being admitted to hospital on the second occasion i was told i had a complete blockage in the last part of small intestine where it joins the large intestine and had to have major surgery to have it all cut out along with my appendix. I was told after surgery that it was tumorous and i had 18 lymph nodes removed of which 6 were cancerous - likely to be stage 3 - told they couldn't be 100% sure they got it all but wanted to do 8 cycles of chemo as a sweep up to help to give me the best chances of getting rid of it.   

Finally started chemo begin August as had to wait for my wound to completely heal first. I was put on Oxaliplatin / capecitabine (CAPEX) 100% - cycle 1 & 2 seemed okay with the basic side effects of this drug being pins & needles / numbness and advised to avoid touching or drinking anything cold. Cycle 3 hit me for six and i was really lethargic / tired / life less etc and bloods showed my platelets / neutrophils were very low and I was forced to have an additional 3 rest weeks until it was safe to continue. My chemo was reduced to 80% and continued on 11/11/22 then after 1 week i had a terrible tummy ache / back pains and was sick and went to a&e. Bloods taken / pain relief / CT scan was given and was told they ruled out Ischemia & ileitis being the cause for this outburst and was told more likely to be adhesions (scar tissue from op). I was kept in for observation and finally discharged but not long after leaving was called to return as docs had missed the 2nd addendum of the CT scan which mentioned ileitis and the oncology team wanted to see me.

After seeing the oncology team they said the CT was showing that it was ileitis (inflammation of the intestine) and they advised me to stop taking the rest of the tablets for cycle 4 as a precaution.  After seeing my chemo consultant on Tuesday he said there is a risk if i continue with the chemo it could make the ileitis worse which could cause issues in the future. If i stop the chemo now i am also not giving myself the best outcome than if i complete all 8 cycles. If i continue they will reduce the chemo down to 50%. So i am in a bit of a dilemma and having to make a big decision.......just wondered if anyone has been put in a similar position and any words of advise?

  • If the Chemo may cause your present problems to get worse, I would say give it up for the time being, until it settles, & you start to feel better. Once you feel stronger, resume the chemo treatment.

  • Would it be possible to swop off the tablets? Then the intestines might not get so inflamed? Just a thought! Sending ((hugs))

    Your story is identical to me until you get to the inflammation part I never had that however, the Oxaliplatin they reduced with me as I wasn’t tolerating the dose they had me on.  I went through everything to the letter as you did. I had the adjuvant treatment and have been clear of cancer for 10 years. Unfortunately it has returned a few months ago. 

    If they are advising you it’s ok to have a reduced amount because of the inflation I would take it but clearly this has to be your decision along with consultation with your oncology team.

    PS. I was also diagnosed with Crohn’s disease at the same time as bowel cancer.

  • unfortunately, been told if i decide to stop then the chemo wont be able to restart and that will be the end of it .

  • I asked if there was an alternative chemo drug i could change to but been told no. I have questioned this and told by mcmillan nurse that its not as easy as that as i thought probably was the case   

  • Hi, really sorry to hear your cancer has returned. They are not advising either way as it's my decision. They cannot tell me how severe the ileitis is from the ct scan so no idea if its mild or severe. I have asked if i decide to carry on with cycle 5 and if the tummy ache issue happens again and i stop immediately what impact his would have but again they cant answer.

    Yes i understand that ileitis is very much connected to Crohn's disease and sorry to hear you have this as well. Its bad enough being diagnosed with cancer let alone Crohn's as well. Take care & Look after yourself

    All I can say is I had the adjuvant treatment and I had Crohn’s and I was ok. 

    I was stage 3 it gave me another 10 great years and I have no regrets. I plan to have another 10.

    I had small bowel cancer 10 years ago had a right hemi colectomy followed by adjuvant chemotherapy treatment. I had a stage 3 cancer with 3 lymph node involvement. Diagnose with crohn’s at the same time. 

    This is the first recurrence I’ve had. Diagnose end August beginning September. 

    No idea how they are going to treat me as I’ve been advise no treatment at present as I have no symptoms. 

    Was found after having a follow up Ct scan on kidney surgery I had the year before.

  • I was unsettled when they reduced my chemo dose and the nurses (or maybe it was the onc) reassured me that when initially doing trials they always go for the maximum tolerable dose in humans as the usual dose, not the minimum dose required to kill cancer cells in a petri dish.....it would be unethical to try to do trials on a reducing dose to see when the chemo drugs are no longer strong enough to have an effect in humans. you wouldn't  want to get that research arm ;)      ..... so for all we know 50% could be just as effective as 100% dose just more easily tolerable? ... i really don't know that validity of that hypothesis however it would be enough for me to keep going and give yourself every chance. if at 50% you still have issues think again. JMHO
