Working while caring

Hi,

My self and my mum are currently caring for my uncle he has no one else his stage 4 lung cancer is very advanced my question is I work nightshift Monday-Friday so it’s very hard fitting in helping mum he has carers morning and night but becomes extremely agitated when they are in as he prefers us to do his personal care which is difficult for my mum if I’m sleeping could I get a fit note from the doctor in this situation or is there anything else I can do my employer had said I’d need to take unpaid leave but this is a no as I have other financial responsibilities can anybody help please?
