Help! Has anyone had to move their Oncology care from Australia to the UK?

Hi, may be a long shot but my husband was diagnosed with oesophageal cancer in July 2023. We have been living in Australia so he was diagnosed here, and had treatment and surgery in January 2024. Unfortunately, his recovery has not gone to plan and the cancer has returned. He is deteriorating quickly and has been told he has less than 12 months. He said he wants to move home and potentially continue treatment in the UK but definitely end of life. How do I get him integrated back into the NHS? I have registered him with the local GP in the area we will move back to but I need to open up a line of communication so his oncologist here in Australia can transfer his medical records etc. I'd like to try and get this started now for him so when we are back the GP knows the situation and a referral to an oncologist can hopefully be in process. Currently I'm not having any luck communicating with the Medical Centre I have registered him. I have sent emails and tried to call. I was going to get a family member to go in on our behalf but my question is... Is there a process? we can't be the first people trying to do this. It feels too scary to just up and leave Australia with nothing at all in place when we arrive. Any advice would be so welcome. Thank you anyone in advance that is reading this.
