Newly diagnosed, I want to get my life organised. My GP tells me my old Advanced Directive got lost when the went digital..does Macmillan have a downloadable template for this before I search elsewhere?
Hi and welcome to the Online Community, although I am sorry to see you finding us and so sorry to hear that you have just been diagnosed.
I had a look using the search tool on the main Macmillan site and there is lots of info about Advanced Directive but could not see a link to downloadable forms. But as we have just done this for my mum the forms are available on the UK.GOV website.
You don’t say what type of cancer you have been diagnosed with - we do have lots of support groups that are Cancer specific and also ones that are general Cancer experience groups.
Follow the links above, have a look and join the group(s) by hitting the ‘Join the Group’ tab just under the main group name and it is worth indicating how you want to receive email notifications when someone answers your posts.
Go to the 'Start a Discussion' tab just under the main group name and set up your own Discussion and introduce yourself to the group.
You may find our various Macmillan Support Line Services to be helpful - call them on 0808 808 00 00This free service covers Emotional Support, Practical Information. Clinical Information, Financial Support and Work Guidance mostly open 8.00 to 8.00 but check the link.
We also have our ‘Ask an Expert’. section where you can post questions to our mostly Volunteer Experts but please allow 2 working days to get a reply.
Our Online Information and Support Section is a good place to find information covering cancer diagnosis, treatments and pages covering most types of cancers.
When you feel up to it try putting some information in your profile. This really helps others when answering. It also means that you don't have to keep repeating yourself. Just click on YOUR username, select 'Edit Profile'. Put as much or as little in your profile and you can amend it at any time - you can see members profiles by hitting our forum names.

Hi there..Many thanks for your swift reply.
There doesn't appear to be a link, so I went to NHS site and followed a link on there, a friend is printing it for me and I'll complete it tomorrow and photocopy Monday, for distribution. That's the first on an unimaginably long 'to do' list, before the op renders me incapable of doing much for weeks!

There is always the lasting power of attorney section on health but it is more legal and needs to be registered and can be costly depending on the way you go to prepare one, but no matter which way you complete the LPoA forms there is a cost for registering each form for health and finance.
Apologies if this has arrived too late for you and you've already got everything in hand but it may be of help to others.
Let's hope you never have to use your decision for a very long time.
