Pseudoprogression anyone familiar on Immunotherapy

Hi All hope your all keeping well and baring up on this trip. I saw my oncologist on Friday and he said my liver tumour has grown from 15 cm to 17 cm and  my lung tumours have grown by one or two mm . He mentioned Pseudoprogression which I understand is growth before hopefully stopping . He said if it was  chemo I was on they would stop but because I am on Immunotherapy it's ok . So in limbo on this one and will have treatment next week and then another scan in 6 weeks for verdict. On an up note the tumours in my neck are stable so Happy Days there lol 

  • Pseudoprogression is commonly seen on the first scan after starting immunotherapy. The explanation is that when immunotherapy works, T cells invade the cancer, and this initially makes the whole mass look bigger. When they see the second scan it will either have shrunk (if the immunotherapy is working), or they will know it is real progression. It happened to me on Pembrolizumab. 

  • Hi Coddfish  Thanks How are you doing now  ? .This was my first scan  after 3 sessions of Opdivo . Fortunately I had read about it but just didn't expect him to tell me my tumours had got bigger. Am I right in thinking even if it gets bigger they will give me 6 months treatment as that's how long this progression can go on before it shrinks.  

  • That’s what I was told, albeit on a different immunotherapy (Pembrolizumab). In my case the 3 month scan had one smaller, one larger, but the 6 month scan showed material reduction on both. I am doing well cancer-wise (have been in remission for a year) but lots of other consequences from the immunotherapy. 

  • Hi I to had growth after first one, my oncologist told me, we will give you three infusions, then see what's occurring, every thing did settle down and in the end, i completed the two year course.

  • Thanks Coddfish That is good news regarding remission  unfortunately there is no getting away from the side affects. I have had radio once  chemo twice and  neck dissection , so use to side effects . 

  • Hi Ellie  Thanks Well infusion no 4 next week and then oncologist in a months time and then scan is the play I think . Yes I am keeping my fingers crossed lol . All the Best Regards Minmax
