Hi, I wondered if anyone has completed an HC50 form related to their cancer diagnosis. My husband finally agreed to try to claim Universal Credit, he was diagnosed in April last year and we have struggled until now. Long boring story!!! Anyway, he received an HC50 to complete, however, it seems to suggest that he only has to complete the first part that deals with his personal details and 3 questions related to his diagnosis then get his oncologist to complete section 24, is this correct or are we reading it incorrectly?
Any advice would be very welcome. We are total newbies, having never claimed any form of benefits before.

I wonder if the best thing I could suggest is to give the Macmillan Line a call and speak to one of their Information Advisors who may be able to help with this query. 08088080000. Alternatively if you have a Citizens Advice Bureau near you or contactable by phone give them a call as they will also can help. I notice you’re a member of the prostate group, so perhaps trying posting there too to see if any of the members have experience they can share? It can be good to speak to others who might have had to do the same thing and might be able to advise.
