Multiple Myeloma relapse?

I am 77 years old and was diagnosed with Multiple Myeloma in June 2026 and underwent 6 months induction with quad therapy. My Kappa value went down 85% by cycle 6, and I was told that the response was sub-optimal and that I needed a transplant (already scheduled for February).

Two weeks after stopping the all medications (prior to stem cell collection)  my Kappa went up from 20 to 45.

Does this mean a relapse (aggressive disease, poor prognosis) or is it normal if you don't follow induction immediately with maintenance?

I'm nervous about it and won't talk to my oncologist (at MSK) for another 3 weeks.

    It’s difficult for us to say why your Kappa levels have increased.

    I appreciate that it is some weeks before you can talk with your oncologist, but it’s best that you talk with them. They have access to your medical notes and history, and they will be able to clarify the possible reasons for the increase.

    If you have a contact number for a haematology clinical nurse specialist (CNS) at your hospital, I encourage you to contact them to talk this through.
