PERT and other drugs not working

Not only is my friend who has pancreatic adenocarcinoma struggling with PERT maldigestion and malnutrition but also zoromorph, codeine, lanzoprozole, antiemetics and imodium seems to be ineffective due to chronic steatorrhoea and diarrhoea. I presume that none of these drugs are being absorbed properly due to to rapid transit time. It's a catch 22 situation as the imodium needs to work to stop the diarrhoea, the codeine for the same and pain along with the morphine and the lanzoprozole is not inhibiting gastric secretions enough to enable PERT to work. Perhaps to reach a threshold of drug activity everything needs the dose titrated upwards to be therapeutic. Some advice would be welcome as she has lost over 20kg since August (100g per day). I tried today to speak to CSN but unavailable, so too the senior dietitian and a message was sent to the oncology team to give me a call back but not heard anything. It will be another 300g before Monday. Oncology consult on 10th Jan. Please help

    It does sound that your friend is going through quite a time, and it’s understandable that you are trying to get her help with the symptoms you mention.

    Unfortunately, as a support line team we have no access to any clinical records, and as we have no direct involvement with individual cases I’m afraid we are unable to offer specific advice about medication and symptom management.

    As your friend’s situation is quite complex changes to her treatment should be overseen by someone who knows her, however over the weekend she can call 111  for advice.

    I’m sorry that you were unable to speak with anyone from her team yesterday, and hopefully someone will be in touch at the start of next week. However, it would be a good idea to call them again on Monday to let them know of her situation, especially if she calls 111 and they recommend changes to her current plan.
