Mastectomy / Reconstruction 2 Incisions or Failure of Implant

Hi

Had a single mastectomy 22 June with temporary expander implant reconstruction at the same time.

due to dressings and drains and  mirrors only for heads assumed I had only 1 incision across the breast about  8” horizontal. Had 1 drain removed 2.5 weeks post op but 2nd remains and out put has on average 60 mls, but like yesterday it was 120mls.

the bottom of breast (skin sparing as well) the ridge from the body to the breast has been hard since surgery and over the 10 days has softened to palpating in some areas. Took a selfie this morning after a bra free night - and noticed about2.5 to 2cm in from body on the breast there is a line extending across the breast

not sure if it is a 2nd incision line that I wasn’t aware of before or if it is the harder plastic of the temporary expander implant. I know there is no sling for my my implant due to a large section of skin being removed from the breast medial to lateral due to surgeon concerns

I have left message with the breast care nurses to call me back at 9 this morning and as it is 4:15 assuming they won’t be calling back fence the questions here. Secretary or surgeon aren’t in today

quick summary

1) is it normal to have 2 incisions (2) concerned about that the implant is failing if it’s not a 2nd incision (3) what are the signs of implant extrusion (I have looked this up but they talk about seeing the implant and the suture line breaking down - the medial to lateral mid breast sure line is healing well)

    Thanks for getting in touch and welcome to the Online Community. I hope you have found it supportive so far. I’m Helen, one of the Cancer Information Nurse Specialists on the Macmillan Support Line. I am sorry to hear that you are having concerns following your surgery.

    The second line you have found since you returned home may be an incision or the expander implant, but we are unable to say for sure. I want to reassure you that you would have been examined prior to discharge from hospital and would have been informed if your team had any concerns about how the surgery went.

    You don’t mention any pain, redness, heat or leakage from the wound that would indicate there was a problem with wound healing. Meaning it is very unlikely the implant is failing.

    With regards to possible extrusion, this is quite rare and only occurs of there is breakdown of the scar line.

    You’ve done the right thing contacting your team about these concerns. They will have access to your operation notes and be able to explain the things you have mentioned. If they have any concerns, they will arrange an examination, just in case.
