I was advised to get a DS1500 so I could have Attendance Allowance (I am 68) and a Blue Badge.
The extra money would be nice, but what I really want is the Blue Badge so my drivers can get closer to the hospital entrance.
My hospital team has sent my DS1500 straight to DWP and I can't seem to make them understand that I need a second copy as the Blue Badge is dealt with by the local Council.
I was told that the hospital have a scan of the DS1500 on file and they would give me a copy, but all they have given me is a letter saying that they have sent to DS1500 to DWP.
The council website says they will accept a hospital letter in lieu of a DS1500 if it has the same information, but the one the hospital have given me says practically nothing.
Does anyone know what a DS1500 says - I can't seem to find an example anywhere on the web?
The council website also says the waiting time for Blue Badge applications to be processed is 12 weeks. I don't want to send the letter I've got then find it rejected in 12 weeks and have to start again. I may not be alive that long.

Many thanks Jane, I'm sure it will be about the same information, if not design. I found one reference that hospital professionals can now fill in a DS1500 online, so maybe they just don't have a copy anymore. I will have to nag again on Monday.
Do you have a specialist nurse? She should be able to help you or possibly access a Macmillan CAB person. In my area, you can get in contact with a citizen advisor who helps you fill in the dwp and blue badge form, coordinating the ds1500 form too.

how very frustrating! My hospice handled all that for me so I can’t help. You might find the financial advisers on the Macmillan helpline can help - the number and opening hours are on the Macmillan home page.
Good luck! My blue badge makes such a difference to my life. Being able to park at the station when I have to travel to hospital or near the shops so I don’t have to walk that extra 100m makes trips possible rather than impossible.

I'm not sure that your council would supply your blue badge. I would also advise you to contact MacMillan or CAB. The MacMillan representative at my local hospital filled in my Blue Badge application over the phone, asking me the questions then she sent me the form to sign and I posted it to a government department but I'm in Northern Ireland.
I remember I needed it in a bit of a hurry for something and I phoned them and spoke to a very very pleasant lady who was very accommodating. Although she as much as promised me she would fast track my application, she didn't say it outright but my blue badge was delivered within a week.

Hi ,
When I applied for mine I had to send evidence that I had been awarded PIP. I received the letter very quickly from the DWP once the DS1500 was received by them and could then get my blue disc.
For my renewal of PIP recently I called in at the little Macmillan office in my local hospital and it was sorted in a couple of weeks from start to finish. As I already have my blue badge, free bus pass and disabled railcard I didn't need any help with this but know that they do offer this assistance as well. At my hospital it is a walk in service although if you need to see a financial advisor you need an appointment. Does your hospital have a Macmillan office where you could get some assistance?

I live in one of the villages between Leeds and Harrogate. Leeds CIty Council forget the villages have a Leeds postcode . After a referral by the nurse specialist , Leeds sent my documentation to the Macmillan welfare advisor who is based at Harrogate Hospital. He was brilliant. He sent me the forms for PIP, Macmillan can help you with the DS1500, which helps to speed the process up. I was awarded PIP within three weeks . I also have a Blue Badge, sent the forms back and received the badge within 4 weeks. I would suggest a chat with the welfare advisor at Macmillan or a Maggie’s centre if there is one near. Hello all and thanks. It is the nurse specialist that doesn't seem to understand that the Blue Badge and Attendance Allowance are arranged by different people.
Gov.uk website is quite clear that my local council do our Blue Badges, although the online form in on the gov.uk website. The council website is also as clear that I need evidence of illness, treatment and prognosis. I understand how to do the forms, I just need the evidence. Maybe I will eventually get a letter from DWP which will have the required information, but I was hoping to get on with it asap.
There is a benefits lady associated with my local hospice palliative care service. I believe she works for both CAB and Macmillan. She may be a good bet to ask as she will probably know what the local council accept. The council say they have a 12 week processing backload and any queries will slow it down even more.
It will probably be difficult to get hold of anyone this week, but I will try.
