Ds1500 and pip

I have a ds1500 in place which my specialist nurse filled in for me which entitles me to the higher rate of pip which is due to run out soon does anybody know if I have to get another ds1500 ? Or do they use the one I have in place when I reapply for pip.would like some help on this subject please.

    I think they do a new one due to the information on it and timescales.  I can't imagine it would be an issue.  Speak to your nurse to fill in the necessary

    As Daisy says, you'll need a new one. The DS1500 basically says, and I'm sorry for being blunt but there's no other way to put it, that the doctors don't expect you to live longer than 6 months. So if after 6 months you're still around, then you need another one - but you'll only get it, or should only get it, if they still think you've got less than 6 months left.

                  Thanks for the info yes i am aware of the timescales 6_12 months but that's only an estimated guess  as no one can truly say how long we have left I've seen posts on cancer survivors on Facebook saying they have re applied for it after 3 years but not if they had to send in another ds1500 so I was just curious to know if that's the case I will get my specialist nurse to fill another form in for me thanks for your help though much appreciated.

    Hi Lass sorry for butting in but the award usually lasts for three years not six months. I am six years diagnosed and have had to renew with a new ds1500 every three years. I will have to get a new one this year.xx All the best. Lynn 2

    The award does, yes. The DS1500 doesn't as it is filled out to say someone only has 6 months left max.

    So the way I read the OP, was that they'd had a DS1500 and got the award. The award now needs renewed, and so I said they'd need a new DS1500 if those conditions are still relevant. So exactly the same as you just said too. 

    If it doesn't look like they've only got 6 months, or less, left, then they'll have to go through the horrific process I'm in the middle of, because they don't qualify for a DS1500.

    Thanks for your help on this subject it has cleared it all up for me now so basically I have to get another ds1500 after 3 years

    Hi. Just been through the same. I knew mine was coming up to renewal. I just spoke to my specialist nurse who without hesitation said they would complete and send on my behalf. They also sent the patient copy to me which was useful as dwp, when i enquired, claimed to have not received the original. They then accepted a photocopy of my patient copy and it was processed accordingly.
